What?

Why?

Dam failureConstruction defects in the clay core or drainage zones cause seepage that leads to catastrophic dam failure.
Plant overturnHeavy dump trucks and compactors operating on steep embankment slopes overturn, crushing operators.
Drowning riskRising reservoir levels during construction and proximity to deep water create drowning hazards for the workforce.
Do Don't
  • Follow the fill placement specification exactly for layer thickness and moisture content.
  • Compact each layer to the required density and test before placing the next layer.
  • Maintain safe gradients on haul roads and restrict plant to approved routes on slopes.
  • Wear a personal flotation device when working near the impounding reservoir.
  • Place filter and drainage zones in the exact sequence shown on the design drawings.
  • Protect exposed clay core from rainfall with sheeting when work stops.
  • Report any seepage, wet patches, or unusual conditions on the embankment face.
  • Follow the supervising engineer instructions for all construction stage inspections.
  • Record all compaction test results and layer placement data for the dam safety file.
  • Maintain rescue equipment at the reservoir edge throughout the construction period.
  • DON'T place fill material that does not meet the specified moisture content range.
  • DON'T skip compaction testing on any layer before placing the next one above.
  • DON'T operate heavy plant on embankment slopes steeper than the approved gradient.
  • DON'T work near the reservoir without a personal flotation device.
  • DON'T place filter or drainage material out of the designed sequence.
  • DON'T leave the clay core exposed to rain without protective covering.
  • DON'T ignore seepage, wet patches, or soft areas appearing on the embankment.
  • DON'T deviate from the supervising engineer instructions without formal approval.
  • DON'T use unsuitable fill material that has not been tested and approved.
  • DON'T rush construction to meet programme at the expense of specification compliance.
