Confined Space Entry Permit Procedures

A confined space entry permit is a formal safety document that must be completed and authorised before anyone enters a confined space. The permit ensures all hazards have been identified, controls are in place, atmospheric testing has been carried out, and a rescue plan is ready. It is not simply paperwork — it is a systematic check that the conditions for safe entry have been met. Entry without a valid permit is strictly prohibited.

Control Measures
  • Obtain a signed confined space entry permit from the authorised person before any entry.
  • Verify that atmospheric testing has been completed and readings are within safe limits.
  • Confirm all mechanical, electrical, and pipework isolations listed on the permit are in place.
  • Ensure a trained top man is stationed at the entry point with communication equipment.
  • Check that rescue equipment including tripod, winch, and breathing apparatus is ready for use.
  • Brief all entrants on the permit conditions, hazards, duration, and emergency procedures.
  • Display the permit at the confined space entry point so it is visible to all personnel.
  • Monitor the atmosphere continuously during the entry and evacuate if readings change.
  • Close out the permit formally when work is complete and all persons have exited safely.
Emergency / Rescue

If the atmosphere changes or a worker shows signs of distress, evacuate the space immediately. Activate the rescue plan. Call 999. Never enter to rescue without breathing apparatus and a trained rescue team.

Remember
  • No entry to any confined space is permitted without a valid, signed entry permit in place.
  • Atmospheric testing must be completed immediately before entry and continuously during work.
  • All isolations listed on the permit must be confirmed in place before the first person enters.
  • The permit must be displayed at the entry point and visible to every person involved.
  • If conditions inside the space change during work, evacuate immediately and inform the permit issuer.
  • The rescue plan must be tested and all rescue equipment checked before entry begins.
