LOTO Awareness and Legal Requirements

Lock out tag out is a critical safety procedure that prevents the unexpected start-up of machinery, plant, or systems during maintenance and repair work. Failure to properly isolate energy sources has caused fatalities on construction sites from electrocution, entrapment in machinery, and exposure to pressurised systems. LOTO ensures that all energy sources are identified, isolated, locked, and verified as dead before any work begins.

Key Hazards
Electrocution from unexpected re-energisation of electrical systems
Entrapment or crushing from machinery restarting during maintenance work
Scalding or chemical exposure from pressurised pipework releasing unexpectedly
Gravity hazards from suspended loads or raised components dropping without isolation
Control Measures
  • Identify all energy sources including electrical, mechanical, hydraulic, pneumatic, and gravitational.
  • Isolate every energy source using the designated isolation points for each system.
  • Apply personal locks and tags to each isolation point with your name and contact details.
  • Verify that the system is dead by testing with a proven voltage indicator or operation attempt.
  • Use a multi-lock hasp where more than one person is working on the same isolated system.
  • Follow the site-specific LOTO procedure and ensure it is documented in the permit to work.
  • Never remove another person's lock or tag without following the formal override procedure.
  • Ensure all stored energy such as capacitors, springs, and pressurised vessels is safely dissipated.
  • Communicate with all affected personnel before isolating or re-energising any system.
Emergency / Rescue

If a system unexpectedly starts up during maintenance, move away immediately and raise the alarm. Do not attempt to shut down equipment while in the danger zone. Call for the authorised person to re-isolate.

  • Always apply your own personal lock — never rely on someone else's isolation.
  • Verify the system is dead after locking out and before starting any work on it.
  • Never remove another worker's lock or tag unless authorised through the formal procedure.
  • All energy sources must be identified, including stored energy that may not be obvious.
  • A proper LOTO procedure is a legal requirement under PUWER and electrical safety regulations.
