Lock out tag out equipment including padlocks, danger tags, and multi-lock hasps are the physical devices that prevent accidental re-energisation during maintenance and construction work. Using the correct LOTO equipment and applying it properly is essential to keeping workers safe from unexpected energy release. Control Measures
  • Apply a personal padlock to every isolation point before starting any maintenance or installation work.
  • Use a unique, individually keyed padlock that only you hold the key for.
  • Attach a completed danger tag to each lock clearly stating your name, date, and reason for isolation.
  • Use a multi-lock hasp when more than one person needs to work under the same isolation.
  • Ensure every person working on the isolated system applies their own lock to the hasp.
  • Never use another person's lock or remove anyone else's lock from an isolation point.
  • Remove your personal lock only when you have confirmed all your work is complete and safe.
  • Store LOTO equipment in a dedicated, accessible location on site with sufficient supplies.
  • Brief all operatives on the LOTO procedure and the correct use of locks, tags, and hasps.
